Job summary

Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to deliver on-site, team-based primary care within Assisted Living and Memory Care communities in and around Blaine, MN. You will join a growing practice with a flexible schedule supporting patients Monday through Thursday and optional Friday clinical or administrative duties.

You will manage a panel of about 200 patients, design individualized care plans, and collaborate with LPNs, RNs, and social workers.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certification/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification for the state of work.

Responsibilities

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full-time providers care for patients Monday-Thursday; option for Fridays as clinical or admin day.
• Manage a panel of approximately 200 patients (roughly 12–16 per day, 14 on average).
• Design and manage your schedule.
• Develop and implement care plans tailored to patients’ desires.
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers.
• Access to diagnostics such as labs and x-rays as needed.
• Collaborate with community partners within home care, hospice, and other resources.
