Job summary

Sweetwater Care in California seeks a licensed LVN to provide nursing care within scope of practice, support quality improvement, and assist in patient education and coordination of care.

The role requires California LVN licensure, preferred nursing management background, and long-term care experience with familiarity in Point Click Care charting. On-site position with participation in facility policies and procedures.

Qualifications

• Licensed LVN (CA) or compact license if applicable.
• One year nursing management experience preferred.
• Long-term care experience and Point Click Care charting experience.

Responsibilities

• Uses Infection Control Manual and adheres to policy.
• Participates in Quality Improvement Program.
• Utilizes Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admit, discharge).
• Educates patients and families.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives and carries out physician orders per Nurse Practice Act.
• Might make rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred

• POINT CLICK CARE CHARTING EXPERIENCE
