Job summary

Sweetwater Care in Modesto, California is seeking a qualified LVN Nurse Manager to lead nursing operations and ensure patient care meets standards of practice.

You will administer medications and treatments per policy, participate in quality improvement, and coordinate with physicians. Strong charting in PointClickCare and long-term care experience are essential.

Qualifications

• Licensed LVN in California or compact license required.
• Minimum 1 year of nursing management experience preferred.
• Experience in long-term care and Point Click Care charting is required.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in implementing the Quality Improvement Program.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
