Job summary

Colossal Health & Rehab Center in Worthington, OH seeks a dedicated professional to support the Social Services Department and coordinate resident care plans. You will assist in planning, implementing, and evaluating services to meet emotional and social needs in compliance with regulations.

Position Summary:

The primary purpose of your job is to assist the Director to ensure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. You will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the community’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures.

Education/Qualifications:

• Must possess a high school diploma or equivalent.
