Position: Nurse Chronic Care Coordinator, Remote

This role involves telephonic encounters with patients on behalf of our partners each month and developing detailed care plans within our electronic health record templates. Initially, this is an Independent 1099 Contractor position, with potential to transition to full-time W2 employment with benefits.

Esrun Health seeks Nurses to work part-time from home, complying with HIPAA privacy laws. You will set your own hours, with a minimum commitment of 20 hours per week. Flexibility allows you to choose your work days and hours. Typically, part-time team members dedicate 20-30 hours weekly to their assigned patients.

Patient Panel and Responsibilities

• Manage a patient panel based on skill and efficiency, with a minimum of 100 patients per month.
• Complete encounters on 90% of assigned patients.

Compensation

We pay per completed encounter, with tiered rates based on volume:

• $10.00 per encounter up to 99 encounters/month
• $10.25 per encounter for 100-149 encounters/month
• $12.00 per encounter for 150-199 encounters/month
• $14.00 per encounter for 200-249 encounters/month
• $16.00 per encounter for over 250 encounters/month

Payments increase require two months of consistency. Each encounter takes at least 20 minutes, including all related tasks.

Qualifications

• Graduate from an accredited Nursing program (LPN, LVN, RN, BSN).
• Valid license with no disciplinary actions.
• At least two years of clinical experience in Med/Surg, Case Management, or home health.
• Experience with Electronic Medical Records and Microsoft Office 365 in a HIPAA-compliant home environment.
• Strong initiative, judgment, organization, and problem-solving skills.
• High-speed internet; Windows 10 or higher or Mac OS (no Chromebooks/iPads).
• Excellent communication skills.
