Job summary

Sweetwater Care is seeking an LVN with management experience to lead nursing operations in a California long-term care setting. The role emphasizes adherence to infection control guidelines, active participation in quality improvement initiatives, and close communication with physicians and care teams.

Responsibilities include supervising medications, treatments, and orders, educating families and patients, coordinating patient changes, and ensuring policy compliance in all daily activities.

Qualifications

• Currently licensed as LVN in California; consider compact license if working in a compact state.
• One-year nursing management experience preferred.
• Experience in long-term care settings.
• Experience with Point Click Care charting.

• Participates in and assists in the Quality Improvement Program.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely and according to facility policy.
• Administers treatments timely and according to facility policy.
• Receives, transcribes, and carries out physician orders per Nurse Practice Act where allowed.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.

• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
