Job summary

Sweetwater Care is seeking an experienced LVN supervisor in California to ensure high-quality nursing care within scope of practice and facility policy. You will support physician orders, participate in quality improvement, and educate patients and families.

The role requires an active California LVN license and preferred long-term care experience, with familiarity in Point Click Care charting. Expect rounds with physicians and adherence to all regulatory guidelines.

Qualifications

• California LVN license is required; compact license acceptable in a compact state.
• 1 year of nursing management experience preferred.
• Long TERM CARE experience is preferred.
• Experience with Point Click Care charting is required.

Responsibilities

• Adhere to Infection Control Manual and center policies.
• Participate in Quality Improvement Program.
• Utilize Quality Life Manual for patient placement and monitoring.
• Notify departments of patient changes (room changes, LOA, admits, discharge).
• Provide family and patient education.
• Administer medications timely and according to policy.
• Administer treatments timely and according to policy.
• Receive, transcribe, and execute physician orders as allowed by Nurse Practice Act.
• Conduct rounds with physicians as needed.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
