Job summary

Sweetwater Care is seeking an LVN in California to supervise nursing staff within the long‑term care setting. The role emphasizes adherence to infection control practices, quality improvement, and coordinated patient care.

You will educate families and patients, administer medications and treatments, and collaborate with physicians. The position requires CA licensure and prior management experience, with proficiency in Point Click Care charting.

Qualifications

• Currently licensed as LVN in California; compact license if working in a compact state.
• One year of nursing management experience preferred.
• Long-term care experience required.

Responsibilities

• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Uses Quality Life Manual to identify, place, and monitor patients in programs.
• Notifies departments of patient changes (room changes, LOA, new admits, discharge).
• Conducts family and patient education.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes, and executes physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
