Job summary

Capital Caring Health in Aldie, VA is seeking a Per Diem Registered Nurse to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Group (IDG), and provide palliative and supportive care in home, hospice, and facility settings. You’ll ensure safety, comfort, and high-quality care across shifts.

Required are an active RN license in the state, BLS, EMR/EHR experience, and a valid driver’s license with reliable transportation.

Qualifications

• Current RN license in the state of employment
• Active BLS for healthcare professionals (AHA/Red Cross)
• Minimum 1 year inpatient nursing or hospice experience for external candidates
• Experience with EMR/EHR systems
• Valid driver’s license and reliable transportation

Responsibilities

• Provide direct patient and family care within IDG plans
• Coordinate Plan of Care ensuring individualized needs are met
• Educate patients and families about disease, hospice care, and safety practices
• Obtain physician orders and communicate care plans
• Manage nursing interventions to prevent complications and maximize comfort
• Ensure timely documentation and liaise with facilities and physicians
