Job summary

Apploi is seeking an experienced registered nurse to plan, organize and direct hospice care within patients’ homes and communities. The role emphasizes community health education and coordination with the interdisciplinary team.

The nurse will assess patient and family needs, develop and implement care plans, administer treatments, and provide guidance to patients and caregivers while maintaining quality and safety in a home setting.

Qualifications

• Graduate of an accredited school of nursing.
• RN licensed in good standing to practice.
• Experience in professional nursing within the last 3 years preferred in public health/home hospice.

Responsibilities

• Perform initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Deliver professional nursing care per the nursing process.
• Develop and revise plans of care with patient/family involvement.
• Coordinate patient care as case manager when assigned.
• Administer medications and treatments as prescribed.
• Provide patient/caregiver education and health instructions.
• Ensure safe home environment and continuity of care.
• Participate in interdisciplinary care planning meetings.
