Job summary

Hackensack Meridian Health in New Jersey is seeking an Advanced Practice Nurse (APN) to deliver patient-centered care across a hospice/palliative setting, including nights and weekends. The APN will lead clinical assessments, coordinate care with physicians, and participate in interdisciplinary rounds to enhance outcomes.

The ideal candidate holds an NLN/AACN accredited NP program, an NP/APN license, and board certification (AANP/ANCC), with prescriptive authority and ACLS/BLS credentials.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited NP program.
• Maintains clinical competencies through continuing education and practice.
• Experience in a relevant patient care setting as regulations dictate.
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration per NJ law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills including MS Office/Google Suite.

Responsibilities

• Performs accurate history and physical assessments appropriate for patient age and problem.
• Orders, performs, and interprets lab tests and diagnostics with physicians.
• Manages general medical/surgical conditions with evidence-based practice.
• Prescribes medications safely per policies and NJ regulations.
• Provide patient/family education and promote APN practice recognition.
• Adheres to nursing protocols, standards of care, and quality indicators.
• Participates in interdisciplinary rounds and care coordination.
• Mentors APNs and educates nursing staff as clinical expert.
