Job summary

Hackensack Meridian Health is seeking an Advanced Practice Nurse (APN) to provide patient-centered, coordinated care with leadership in clinical practice, collaboration, and education. The APN will work with the hospital team to assess, plan, and implement care for patients with complex health and safety needs.

Experience in hospice and palliative care is preferred. The role requires NJ NP licensure, prescriptive authority, and commitment to best practices in value-based care.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate in NP program.
• NPI credential.
• Prescriptive authority via CDS/DEA under NJ law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient with Microsoft Office and Google Suite.

Responsibilities

• Perform history and physical assessments tailored to patient age and condition.
• Order, perform, and interpret labs and imaging with physicians collaboration.
• Manage medical/surgical conditions with evidence-based care.
• Prescribe medications safely according to hospital policy and NJ regulations.
• Engage in interdisciplinary rounds and care coordination.
