Job summary
Hackensack Meridian Health's Per Diem Hospital at Home Nurse Practitioner/Advanced Practice Nurse (APN) delivers patient-centered, coordinated care, collaborating with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs. Weekend and holiday work as needed; no on-call required.
Requirements include NLN/AACN accredited program, Master's/Doctorate NP, NJ RN/APN licenses, NPI, CDS/DEA prescriptive authority per NJ law, and board certification
Qualifications
- Graduate of an NLN/AACN accredited nursing program.
- Master's or Doctorate degree from an accredited Nurse Practitioner program.
- Maintains clinical competencies through continuing education and clinical practice.
- Experience in a relevant patient care setting (or as regulations dictate).
- National Provider Identifier (NPI).
- Prescriptive authority through CDS and DEA registration according to New Jersey state law.
- Collaborative agreement with physicians on staff.
- Excellent written and verbal communication skills.
- Proficient computer skills including Microsoft Office and Google Suite.
Responsibilities
- Assess, plan, and implement care for individuals with health and safety needs.
- Work collaboratively with the practice/hospital team.
- Deliver patient-centered, coordinated care in a hospital-at-home model.
- Weekend and holiday coverage as needed.
- No on-call required.
