Responsibilities

• Maintain and document electronic MAR/TAR accurately and promptly.
• Administer treatments and medications as prescribed.
• Manage the discharge process and ensure continuity of care.
• Educate residents, families, and staff on care plans.
• Complete incident reports and follow-up as needed.
• Provide discharge education on medications and procedures.
