Job summary

Qualifications

• Responsible for planning, directing and implementing a facility-wide nursing program.
• Leads nursing staff to meet goals, ensure patient care and optimal outcomes.
• Manages patient care assignments in line with staff competencies and patient needs.

Responsibilities

• Conduct rounds to assess resident status and nursing needs.
• Provide direct patient care when required.
• Monitor nursing care to minimize rehospitalizations and ensure quality outcomes.
• Oversee MDS process for accuracy and timeliness in the care plan.
• Communicate with physicians for orders and changes in condition.
• Notify families and coordinate discharge planning as needed.
