Responsibilities

• Participates in performance improvement initiatives.
• Evaluates and integrates evidence-based practice into care.
• Conducts and documents accurate clinical assessments.
• Develops, implements, and evaluates individualized care plans.
• Uses nursing judgment to tailor plans to patient needs.
• Manages patient assignment with delegation based on team skills.
• Develops educational plans for patient and family; documents per standards.
• Supports patient/family rights and informed decision-making.
• Regularly revises care plans as patient health changes.
• Develops discharge plans with community referrals as needed.
• Supports onboarding and transition of new staff.
• Guides professional growth through precepting.
