Responsibilities

• Implement the assessment plan with the physician and preceptor.
• Conduct ongoing systematic assessment of patients/families across physical, emotional, social, educational and functional needs.
• Develop a nursing care plan with identified problems, diagnoses, orders, interventions, goals and discharge plan.
• Participate in the interdisciplinary team to plan and evaluate rehabilitation goals.
• Ensure medical record documentation is complete, accurate and timely.
