Qualifications

• Graduate of accredited program for registered nursing.

Responsibilities

• Collaborate with physician and preceptor to implement the care plan and follow-up for patients.
• Conduct ongoing assessment of physical, emotional, social, educational and functional needs.
• Develop a nursing care plan with diagnoses, orders, interventions and discharge planning.
• Participate on the interdisciplinary team in rehabilitation goal planning and evaluation.
• Ensure medical records are complete, accurate and timely.
