Job summary

Kindred Hospital San Antonio is a LTAC hospital focused on patients recovering from critical illness requiring extended medical care. The RN will plan and deliver care using the nursing process and participate in discharge planning with the care team.

Qualifications

• RN license in state of practice (Required upon hire).
• Postsecondary certificate, diploma, or nursing program completion (Required).
• ACLS (Preferred); BCLS (Required on hire).

Responsibilities

• Provide and supervise nursing care per hospital policies.
• Coordinate discharge planning with patient, family and team.
• Administer medications with the five rights to ensure safety.
• Document admissions, assessments, care plans and progress in the medical record.
• Communicate changes in patient condition to physicians and nursing leadership.
