Job summary

VIA Health Partners, a community-based hospice and palliative care provider, is seeking a full-time Registered Nurse for the Huntersville, NC area. The role emphasizes coordination and delivery of patient/caregiver care with a focus on comfort and quality of life. The ideal candidate holds a Diploma or Associate Degree in Nursing, an active RN license, and CPR certification.

CHPN is preferred. Prior inpatient or hospice experience is valued, along with solid computer skills for charting and care

Qualifications

• Diploma or Associate Degree in Nursing required. Bachelor’s degree preferred.
• Current, active, and unrestricted RN license (compact/multistate).
• Current CPR certification; CHPN preferred.
• Minimum 1 year in inpatient units and 2 years in other nursing roles; hospice/palliative care experience preferred.
• Computer proficiency required for documentation and clinical systems.

Responsibilities

• Coordinate and deliver patient/caregiver care.
• Conduct nursing assessments to manage patient/caregiver needs.
• Develop and update the care plan to ensure uninterrupted care and goals.
• Educate patients and families; answer questions and provide guidance.
• Communicate with IDG, patients, and other health professionals for continuity of care.
• Maintain infection control practices and universal precautions.

Essential Functions

Delivery of Patient Care

• Assumes responsibility for the coordination and delivery of patient/caregiver care.
• Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
• When appropriate, develops, implements, evaluates, and updates the plan of care to ensure care is uninterrupted and goals are achieved.
• Facilitates patient and caregiver independence to the extent possible.
• When providing hospice care, appropriately utilizes VIA Health Partners patient related protocols, standing orders and guidelines.
• Assumes responsibility for conducting effective patient and caregiver education.
• Encourages and allows time for questions and consultation.
• Provides education to patients and families as needed.
• Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
• Maintains open lines of communication with supervisor and keeps supervisor apprised of any complex patient/caregiver issues.
• Applies principles of infection control and follows guidelines for universal precautions in accordance with accepted professional practices.

Interdisciplinary Group

• Attends, participates, and may facilitate interdisciplinary group conferences (IDG) and special care conferences.
• Assumes responsibility for establishing and maintaining effective working relationships with team members.
• Identifies specific patient/caregiver needs and makes referrals to appropriate team members.

Documentation

• Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and the requirements of licensure and certification standards.

Professional and Personal Accountability

• Demonstrates competency in knowledge and skills.
• Ensures that VIA Health Partners professional reputation is maintained and projected.
• Assumes responsibility for professional development and staying abreast of current trends in the healthcare field. Incorporates new information and methods into practice.
• Maintains current professional licensure, CPR certification and CEU requirements.
• Maintains productive and professional relations with external agencies and healthcare providers.
• Exhibits time management skills and ability to respond appropriately to crisis.

Other Duties

• IPU: cleans patient rooms according to procedure, ensures all areas of the facility are clean and appropriately stocked, keeps the family room clean throughout the day, ensures proper chemicals are used and protective equipment is employed when working with chemicals.

Qualifications

• A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.
• A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
• Current CPR required; CHPN preferred.
• Minimum year‑long related experience for inpatient units (IPU) and two years for other nursing positions. Experience in direct care of patients with life‑limiting illnesses required; related experience may include hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, medical surgical.
• Computer proficiency is required.
