Job summary

VIA Health Partners in Fort Mill, SC seeks a dedicated registered nurse to join our hospice and palliative care team. This full-time, day-shift role coordinates patient and caregiver care, conducts nursing assessments, and implements plans of care in collaboration with physicians and the interdisciplinary team.

The ideal candidate holds a nursing degree, an active RN license (compact where applicable), CPR certification, and at least two years of related experience.

Qualifications

• Diploma or ADN required; BSN preferred.
• Current, active RN license; compact state license preferred.
• CPR certification required; CHPN preferred.
• Minimum 2 years related experience in life-limiting illnesses; hospice/palliative care preferred.
• Computer proficiency required.

Responsibilities

• Coordinate patient/caregiver care in collaboration with the interdisciplinary team.
• Conduct nursing assessments to manage patient/caregiver needs.
• Make visits, follow plan of care, evaluate progress, and revise interventions as needed.
• Develop, implement, and evaluate plans of care for patients/caregivers; include education.
• Triage emergencies by phone and home visits; communicate with team.

Education

Diploma or Associates Degree in nursing

Bachelor's Degree in Nursing (BSN) preferred

RN license (compact license)

2+ years nursing experience
