Qualifications

• Completion of unit orientation and competency checklist
• Excellent interpersonal communication with patients, families and colleagues
• Certifications as required by unit/area
• Completion of annual competency based learning modules

Responsibilities

• Assess the patient on admission and monitor ongoing status.
• Plan and coordinate care based on assessment and identified needs.
• Involve family/significant others in the plan of care as appropriate.
• Initiate teaching on admission to ensure positive outcomes and discharge planning.
• Administer medications per policy and verify physician orders.
• Collaborate with multidisciplinary team to achieve patient outcomes.
• Advocate for patients and maintain confidentiality and ethical care.
