Responsibilities

• Assess patient needs and discharge planning in accordance with policy and professional standards.
• Plan and coordinate patient care with the multidisciplinary team, including the discharge plan.
• Implement nursing care and supervise activities of licensed staff and other personnel as assigned.
• Evaluate patient progress and revise the care plan as needed; document nursing care in the patient record.
