Maple Health and Rehab in Muhlenberg County, Kentucky, seeks a dedicated RN to join our memory care and rehab teams. You will provide individualized nursing care, coordinate with the care team, and ensure accurate documentation and medication administration.

Qualifications

• Current RN license in the state.
• Up to date first aid and CPR certification.
• At least one year of RN experience in a Long Term Care setting preferred.
• Knowledge of MatrixCare preferred.

Responsibilities

• Provide direct and individualized nursing care based on scientific principles.
• Consult and coordinate with healthcare team to assess, plan, implement and evaluate care.
• Record patient care information accurately and timely.
• Prepare and administer medications and monitor for adverse reactions.
• Educate patients on procedures and post-care.
• Participate in discharge planning.
