Job summary

Gentiva’s Hospice Plus unit seeks an Afterhours RN Care Manager to provide after-hours, weekend, and on-call hospice care. You will assess and coordinate patient and family needs with primary clinical oversight for assigned patients during nights, weekends, and on-call shifts.

The role includes education on hospice care, collaboration with IDG members, and ensuring accurate documentation and compliance with regulatory standards.

Qualifications

• Current RN license in Texas or state of employment.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience in acute care, oncology, hospice or home health.

Responsibilities

• Assess and coordinate patient and family needs during after-hours shifts.
• Maintain primary clinical oversight for assigned patients.
• Educate patients and families on hospice care and disease progression.
• Communicate changes to hospice and attending physicians promptly.
• Conduct supervisory visits and support field staff as needed.
