• Obtains and reassesses patient assessments per nursing policy.
• Develops and revises a plan of care with patient/family.
• Coordinates discharge teaching and documents outcomes.
• Implements and monitors nursing interventions.
• Documents nursing processes in Meditech within required timeframe.
