Job summary

Qualifications

• Current licensure in the State of Ohio or WV license if WV-employed.
• BLS and annual stroke education certification required upon hire.
• ACLS, PALS and TNCC required within 1 year of employment.
• ASLS certification preferred upon hire.
• Stroke Education during orientation and related modules.
• Maintains compliance in RQI.

Responsibilities

• Participates in bedside shift report/handoff.
• Performs hourly rounding.
• Obtains required patient assessments and reassessments per nursing policy.
• Coordinates discharge teaching and documents appropriately.
• Implements and monitors nursing interventions.
• Documents assessments, interventions and outcomes in Meditech within required timeframe.
• Collaborates with interdisciplinary team to identify patient needs.
• Communicates changes with physician using SBAR.
• Triages patients effectively and coordinates care with the team.
• Develops plan of care and teaching plan based on assessed needs.
