Responsibilities

• Assess, plan, develop, implement and evaluate plans of care for specific patient populations.
• Coordinate and collaborate with a multidisciplinary team to facilitate appropriate and timely patient placement or transfer.
• Coordinate and communicate capacity management plan to nursing units, leadership, and physicians.
• Apply clinical knowledge to meet standards of care for patient populations.
