This full-time, day-shift role (36 hours per week) involves coordinating patient transfers across the system with a multidisciplinary team, ensuring timely placement and capacity management.

Ideal candidates have ICU/ED/Transfer Center experience and strong multitasking and documentation skills.

Qualifications

• Registered Nurse (RN)
• Experience in ICU, ED, or Transfer Center
• Recent Acute Care experience
• Ability to multitask, type with reasonable speed and take accurate notes

Responsibilities

• Assess, plan, develop, implement, and evaluate patient transfers across the system network.
• Coordinate with a multidisciplinary team to facilitate timely patient placement and transfers.
• Communicate capacity management plans to nursing units, leadership, and physicians.
• Use clinical knowledge and critical thinking to meet care standards in a fast-paced environment.
