Jefferson Health Ambulatory Clinics in Philadelphia, PA, is seeking an experienced registered nurse to join our ambulatory care team. The role emphasizes patient- and family-centered care, applying the nursing process, and collaborating with providers to deliver safe, compassionate, and evidence-based care.

The successful candidate will maintain PA RN licensure, demonstrate strong communication, cultural responsiveness, and proficiency with electronic medical records while prioritizing quality

Formación

• Graduate of an accredited school of nursing.
• Patient- and Family-Centered Practice Knowledge of how to convey respect and collaborate with the patient and the designated care partner in care and care planning.
• Cultural Awareness Knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.
• Effective Communication Understanding of effective communication concepts, tools and techniques and the ability to convey ideas and needs with patients and families.
• Professionalism Exhibits honesty, confidentiality, and integrity in all activities and communications.

Responsabilidades

• Integrates Evidence-Based Practice with patient/family preferences and cultural beliefs for optimal care.
• Collaborates with clinic care providers to ensure timely communication between PCPs and specialists.
• Supports implementation of clinical care pathways and participates in quality improvement opportunities.
• Documents all patient interactions and nursing care in EMR per organizational standards and regulations.
• Provides patient and family education for preventive care, chronic disease management, and medication management.
