Job summary

Jefferson Health Ambulatory Clinics seeks an experienced registered nurse to provide patient-centered care within the ambulatory setting. You will collaborate with providers and staff to deliver clinical nursing interventions and communicate effectively to address each patient's needs.

Qualifications

Graduate of an accredited school of nursing.
Cultural awareness and effective communication.
HIPAA privacy knowledge and patient confidentiality.
BLS and AHA certifications.

Responsibilities

Integrates evidence-based practice, clinical expertise and patient/family preferences for optimal care.
Collaborates with clinic care providers to ensure efficient communication between teams.
Supports care pathways and participates in quality improvement opportunities.
Delivers education to patients and families on preventive care and disease management.
Documents interactions and care in the EMR in accordance with standards.
