Jefferson Health Ambulatory Clinics seeks an experienced Registered Nurse to support patient-centered ambulatory care. You will collaborate with providers and the care team to deliver comprehensive nursing interventions and address diverse patient needs while maintaining an active nursing license in the state.

The role follows daytime hours (8:00 a.m. – 4:30 p.m.) with no on-call requirements and emphasizes teamwork, education, and coordinated care across the outpatient setting.

Qualifications

• Graduate of an accredited school of nursing required.
• Patient- and Family-Centered Practice.
• Knowledge of how to convey respect and collaborate with patients and designated care partners.
• Cultural awareness and communication skills.
• Ability to document in EMR per standards and regulations.
• RN license in the state of practice.

Responsabilités

• Deliver evidence-based ambulatory nursing care.
• Collaborate with clinic providers and staff for patient management.
• Provide patient education on preventive care, chronic disease, and medications.
• Document all patient interactions in EMR.
• Coordinate care including triage and vaccination verification.
• Participate in team huddles and quality improvement initiatives.
