Job summary

Jefferson Health Northeast is seeking an experienced registered nurse for ambulatory primary care in the greater Philadelphia area. The role emphasizes patient- and family-centered care, coordination with providers, and education on preventive and chronic care.

Strong documentation in EMR and participation in improving care quality are essential. The ideal candidate holds an RN license in the state, has 1+ year ambulatory experience, and BSN preferred.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current RN licensure in state of practice; BLS per organizational guidelines/policies.
• 1+ years of ambulatory nursing experience.
• Commitment to patient- and family-centered care.

Responsibilities

• Provide clinical nursing interventions within primary care.
• Collaborate with primary care providers and staff for coordinated care.
• Educate patients and families on preventive care, chronic disease management, and medication management.
• Document all patient interactions and nursing care in EMR per standards.
• Participate in quality improvement opportunities and care pathways.
