Job summary

South Health and Rehabilitation LLC in Birmingham, AL is seeking a skilled Registered Nurse (RN) to join our long-term care and rehabilitation team. You will provide direct patient care, support rehabilitation plans, and collaborate with physicians and therapists to ensure high-quality resident care.

The ideal candidate holds a valid RN license, strong geriatric and rehab nursing knowledge, and excellent communication.

Qualifications

Valid RN license
Strong clinical skills and knowledge of geriatric and rehabilitation care
Excellent communication and interpersonal skills
Ability to work collaboratively in a team-oriented environment
Compassionate, patient-focused approach to care
Current BLS/CPR certification required

Responsibilities

Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting
Assess, monitor, and document residents' health conditions and progress
Administer medications and treatments as prescribed, ensuring compliance with facility protocols
Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans
Provide education and support to residents and their families regarding care plans and health conditions
Maintain accurate and timely documentation in compliance with state and federal regulations
Foster a safe, comfortable, and therapeutic environment for residents
Supervise and mentor nursing assistants to ensure quality care delivery
