Job summary

South Health and Rehabilitation, part of NHS Management, LLC, is seeking a skilled Registered Nurse (RN) to join our team on the night shift. You will provide direct nursing care, assist with rehabilitation plans, and collaborate with the clinical team to ensure the highest standards of care in our long-term care and rehabilitation facility.

The role requires a valid RN license, strong geriatric/rehabilitation skills, excellent communication, and teamwork.

Qualifications

• Valid Registered Nurse (RN) license.
• Strong clinical skills in geriatric and rehabilitation care.
• Excellent communication and interpersonal skills.

Responsibilities

• Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting.
• Assess, monitor, and document residents' health conditions and progress.
• Administer medications and treatments as prescribed, ensuring facility protocols.
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Provide education and support to residents and families regarding care plans and health conditions.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Foster a safe, comfortable, and therapeutic environment for residents.
• Supervise and mentor nursing assistants to ensure quality care delivery.
