Job summary

Great Plains Post Acute is seeking an experienced LPN/LVN to provide direct nursing care and supervise nursing assistants in a long-term care setting. You will ensure compliance with policies, coordinate care plans, and support discharge planning in a manner that upholds the highest standards of resident safety and quality of life.

Qualifications

• Must possess a current, active LPN/LVN license in the state.
• Must have a Nursing Degree from an accredited college or university or be a graduate of an approved LPN/LVN program.
• CPR certification is required or must be maintained.

Responsibilities

• Direct the day-to-day nursing activities of nursing assistants.
• Supervise and coordinate nursing services to ensure quality care.
• Maintain Nursing Service Procedures Manual and ensure policy compliance.
• Participate in discharge planning and care plan development.
• Complete documentation, charting, and reports in accordance with policies.
• Oversee staffing, performance evaluations, and training of nursing staff.
• Monitor resident care plans and coordinate with physicians and family members.
• Ensure compliance with safety, infection control, and HIPAA requirements.
