Job summary

Decatur Wellness & Rehabilitation Center is seeking a compassionate Night Shift LPN to provide essential nursing care for residents in a team-driven environment. You will direct CNAs, participate in admissions and discharges, and administer medications under physician orders, while ensuring care plans are current and appropriate.

The role requires Tennessee LPN licensure, CPR/BLS certification, and a commitment to confidentiality and high-quality patient care.

Qualifications

• Current unrestricted LPN License for the state of Tennessee.
• Current CPR/BLS Certification.

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess resident’s conditions, prepare, and administer medications per physician orders.
• Initiate, review, and update care plans.
• Manage the nursing care for all residents and delegate shift tasks to nurses.
• Supervise nurse aides as they complete their assignments.
• Observe residents’ conditions, oversee, and respond as necessary.
• Inform physicians of any changes in resident conditions.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.
