The role involves developing care plans with patients and their designated care partners, ensuring alignment with cultural beliefs and values. You will collaborate with an inter-professional team, participate in safety and quality initiatives, and provide education to patients and families

Qualifications

• Specialized Diploma graduate required.
• Knowledge of patient- and family-centered practice.
• Cultural awareness and ability to convey respect.
• Effective communication and partnership with patients and families.
• Ability to assess patients and plan care, including education for patients and caregivers.
• Knowledge of medication administration and discharge planning.
• Experience with health information documentation and clinical data.
