Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required
• Knowledge of patient- and family-centered practice and cultural awareness in care planning.
• Effective communication skills and ability to collaborate with patients and families.
• Knowledge of patient assessment and care planning.
• Knowledge of discharge planning and post-discharge needs.
• Active RN license in state of practice.
• BSN or higher preferred within 3 years of hire
• 1 year RN experience in acute care preferred

Responsibilities

• Interacts with coworkers, patients and families to deliver optimal healthcare.
• Partners with patients and families to identify learning needs and provide education.
• Uses information technology to communicate, manage knowledge, and improve safety.
• Works in inter-professional teams with open communication and shared decision-making.

• Participates in hospital- and practice setting-based safety and quality improvement committees.
