Responsibilities

• Delivers evidence-based nursing care using the nursing process and Relationship Centered Care.
• Assesses patient needs and applies critical thinking to care planning.
• Collaborates with interdisciplinary team to develop, implement and evaluate the plan of care.
• Educates patients and families about plan of care, transitions, health promotion and disease prevention.
• Manages teaching, rounds, discharge planning and delegation within the care delivery model.
