Responsibilities

• Delivers patient care by applying nursing process and professional practice standards.
• Assesses needs and uses critical thinking in patient care management.
• Integrates assessment and intervention skills in nursing care.
• Collaborates with interdisciplinary team to develop, implement, and evaluate the plan of care.
• Prioritizes all aspects of patient care including teaching, rounds, and discharge planning.
• Educates patients and caregivers about plan of care and transition of care.
• Communicates effectively with patients, family and care team.
• Manages assignments within the Modified Primary Nursing model and allocates resources.
• Role models Mount Sinai Relationship Centered Care through Admission Welcome, Bedside shift Report, and other protocols.
• Considers patient values and needs in all aspects of care.
• Establishes and maintains a therapeutic relationship with patient and family.
• Uses evidence-based practices to understand patient perceptions of care.
• Initiates service recovery for patients and family concerns and escalates when needed.
• Demonstrates caring, respect, empathy and active listening.
• Supports staff use of evidence-based practices to improve care perceptions.
• Conducts Purposeful Hourly Rounding on assigned patients.
• Practices safe, patient-centered care and escalates barriers.
• Demonstrates clinical skills crucial to quality and safety.
• Implements process improvements based on evidence and procedures.
• Engages in peer and institutional review processes.
• Collaborates on quality improvement projects and nursing research.
• Contributes to quality and safety practices and compliance.
• Implements high-reliability principles to avoid high risk behaviors.
• Escalates safety hazards following chain of command.
• Documents nursing practice to support quality initiatives.
• Performs charge nurse duties as assigned.
• Coordinates with the Clinical Coordinator/Clinical Nurse Manager to ensure supplies and inventory.
• Escalates material or staffing deficits to charge nurse or manager.
• Demonstrates patient-focused and cost-effective approaches to care.
• Documents accurately.
