Job summary

Ohio's Hospice of Central Ohio is seeking a dedicated Licensed Practical Nurse (LPN) to join Crisis Care Night Shift in Columbus. You will provide compassionate, one-on-one bedside care to hospice patients in private homes and facilities, supporting families during end-of-life journeys.

Under RN supervision, you will monitor patient conditions, document care, and collaborate with the care team to adapt plans.

Qualifications

• Current, unrestricted LPN license in Ohio.
• Current CPR certification.
• Ability to accurately document patient care and services using EMR.
• Valid driver's license and a safe driving record.
• Minimum state-required automobile insurance coverage.
• Ability to drive safely at night, during day, and in inclement weather.

Responsibilities

• Provide high-quality nursing care to hospice patients in private homes and healthcare facilities.
• Deliver one-on-one bedside care to patients experiencing an active decline.
• Monitor patient conditions and communicate changes to the RN and care team.
• Provide comfort-focused end-of-life care addressing physical and emotional needs.
• Collaborate with patients, families, and the hospice team to support individualized care plans.
• Document patient care, services, and observations accurately and promptly.
• Support patients and families with compassion, dignity, and respect throughout the journey.
