Job summary

Buckeye Hospice and Palliative Care Services seeks a licensed Practical Nurse (LPN) to provide skilled nursing care under RN supervision in long-term care facilities and community homes in Ohio.

The role requires an active OH LPN license, 1+ year nursing experience (hospice preferred), and a valid driver's license with insurance. On-call duties, strong communication, and teamwork are essential to support end-of-life care planning and family education.

Qualifications

• Graduation from an accredited School of Practical Nursing.
• Must be licensed in the State of OHIO as a Licensed Practical Nurse (LPN).
• One or more years of nursing experience, hospice experience preferred.
• Must be a licensed driver with an insured automobile in good working order.
• The ability to communicate well, both verbally and in writing.

Responsibilities

• Provide skilled nursing care under supervision of a Registered Nurse.
• Assist the Registered Nurse in carrying out the plan of care.
• Assist the Registered Nurse in performing specialized procedures under physician's orders.
• Assist with preparation, implementation and evaluation of the patient plan of care.
• Collaborate with the IDG to keep the patient’s plan of care current and address issues.
• Reinforce teaching to patient/family about end-of-life care.
• Foster teamwork and participate in staff conferences.
• Pursue self-development through education and on-the-job learning.
• Prepare clinical/progress notes and update Case Manager on changes.
• Perform on-call duties (including holidays) as required.
