Job summary

Gulfside Healthcare Services in Brandon, FL is seeking an experienced Crisis Care LPN to join our hospice-focused team. You will provide crisis care clinical services to patients, and visit home and facility patients as needed, coordinating with RNs and other disciplines to maintain high-quality care.

The role requires FL LPN license, CPR, at least 1 year nursing experience, and preferably hospice/home health or oncology exposure. A valid Florida driver's license and auto insurance are required.

Qualifications

• Currently licensed in Florida as LPN.
• Must maintain CPR certification per AHA.
• Minimum of one year nursing experience.
• Hospice, Oncology, or Home Health experience preferred.
• Minimum of one year acute care nursing with training in symptom management.
• Florida driver's license and automobile insurance required.

Responsibilities

• Assess physical, psychological, social, and spiritual needs and initiate interventions.
• Take independent action to identify and address patient needs.
• Support policies and contribute to staff orientation.
• Mentor and guide co-workers; function as resource person.
• Maintain positive relationships with team members across disciplines.
• Communicate with team RN and Clinical Care Manager regularly.
