Job summary

Chapters Health System in Florida is seeking a dedicated Licensed Practical Nurse to join the Continuous Care team. Under RN supervision, you will perform nursing interventions, provide patient and family teaching, and deliver compassionate care aimed at maximizing comfort and quality of life.

This role requires an active LPN license in Florida, BLS certification, and the ability to travel to patient visits.

Qualifications

• High school diploma or GED required.
• Active LPN license in Florida.
• BLS certification current.
• Excellent English communication.

Responsibilities

• Under RN supervision, provides nursing tasks to patients and families such as continuous care and dressing changes.
• Demonstrates clinical skills in medication administration, symptom management, and patient/family relationships.
• Observes and reports changes in patient condition to RN.
• Performs venipuncture and CVL site observation and care as per protocol.
• Documents per policy in the medical record and supports emergency responses when needed.
