Job summary

Responsibilities include admissions, discharges, wound care, and supervising CNAs and CMTs while following facility protocols and safety guidelines to ensure quality resident outcomes.

Qualifications

• Active LPN license in Missouri
• Night shift availability required
• Ability to document and coordinate care with team

Responsibilities

• Conduct assessments, follow protocol, report changes and document findings
• Perform admission duties: assess, MAR/TAR/POS, chart setup, physician orders, and order medications
• Manage discharges: document, notify physician/guardian, arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and dressings
• Lead CNAs and CMTs, ensure compliance with facility policies
• Respond effectively to emergency situations
