Job summary

Aspen Grove Behavioral Hospital is seeking an Intake Coordinator (RN or Therapist) to facilitate direct admissions from medical facilities and conduct initial patient assessments, nursing evaluations, and care planning. You will collaborate with medical staff and families to discuss assessments, referrals, and levels of care required.

The role includes night shifts on a rotating schedule, comprehensive benefits, and opportunities for career development across UHS’s 300+ locations.

Qualifications

• Master’s degree in social work or other mental health counseling related degree OR a BS Nursing (BSN) REQUIRED
• Unrestricted and valid Utah license REQUIRED
• Minimum 2 years of post-graduate related experience in psychiatric or substance abuse treatment preferred, but not required
• Experience in settings that include inpatient or partial hospitalization preferred
• Familiarity with managed health care process, medical terminology, experience in case management, discharge planning, and/or utilization review preferred
• Must be at least 21 years of age or older and able to pass a thorough background check and drug screen

Responsibilities

• Facilitate direct admissions from Medical Hospitals.
• Perform initial exam of patient, nursing assessment, treatment plan activities for all in-coming and potential patients.
• Consult with potential patients, patient families, medical staff and other hospital staff.
• Counsel patients and patient families with regard to current assessments, recommended referrals and required levels of care.
