The Hospitals of Providence - East Campus is looking for a Full Time Pharmacist to provide pharmaceutical care services. This role involves oversight, monitoring, and dispensing of medications while ensuring compliance with relevant laws. You will be responsible for evaluating patients' medication therapy and collaborating with the care team.

Ideal candidates will possess a Pharm. D. or BS in Pharmacy, with hospital experience preferred.

Competenze

• Required: Pharm. D. or BS in Pharmacy.
• Preferred: Hospital experience.
• Required: Pharmacy license; AHA BLS, ACLS, PALS for direct patient care.

Mansioni

• Review and monitor patient's medication therapy and orders.
• Advise patient care team on medication therapy.
• Verify orders and review medication profiles for safety.
• Ensure compliance with regulations for dispensing medications.
• Check High Risk IVs and initiate/modifies therapy.
