TAG MedStaffing is seeking a Community Outreach Manager in Queens, NY, to lead street outreach, case management, and housing placement services for individuals experiencing homelessness. This leadership role oversees outreach operations and supervises frontline staff.

The ideal candidate is a licensed social worker with supervisory experience, fluent in English and Spanish preferred, and able to collaborate with community organizations and healthcare providers to deliver trauma-informed,

Qualifications

Master's degree in Social Work or related field required.
Active LMSW license in New York required.
2+ years supervisory experience in social services or homelessness services.
Knowledge of Housing First, Harm Reduction, trauma-informed care, and crisis intervention.
Proficient with Microsoft Office; experience with case management systems preferred.

Responsabilités

Program Leadership: Oversee daily outreach operations and supervise Team Leaders.
Clinical Supervision: Provide coaching and structured supervision to staff.
Community Outreach: Coordinate services connecting homeless individuals with housing and resources.
Crisis Management: Lead during critical incidents; support risk assessments and de-escalation.
Cross-Functional Collaboration: Partner with healthcare providers, agencies, and internal teams.
Program Operations: Monitor performance and support compliance and initiatives.
