Job summary

GLIDE in San Francisco is seeking a Journey Home Case Manager to spearhead mobile and on-site outreach, intake, assessment, and wrap-around services. You will develop and manage a caseload of 20–25 clients at various stages toward housing stability.

Shifts are regular (AM/PM/Graveyard) and may include evenings; this full-time role requires a Bachelor's degree in social work or public health, plus de-escalation and motivational interviewing skills. Salary ranges $75,000–$83,000 per year.

Qualifications

• Bachelor’s degree in social work, public health, or 2+ years of outreach and case management to high‑risk populations.
• Experience with de-escalation, motivational interviewing, and data collection.
• Street-based outreach experience in urban settings and with homeless populations.

Responsibilities

• Outreach, educate, and connect people experiencing homelessness to shelter, housing, and wrap-around services.
• Maintain complete case files for 20–25 active clients and submit activity reports.
• Transport clients to appointments and coordinate travel and resources.
• Adhere to HIPAA, ethics, and Glide SOPs to ensure service quality.
• Build rapport and promote Glide services across life domains.
